International Health Certificate Information Form

International health certification information form

  • Please note, if your pet’s Rabies certificate does not meet USDA and destination country requirements, your pet may not be eligible to travel on timely manner. A signed rabies certificate by a licensed veterinarian that includes rabies vaccine name, manufacture, lot number, date and validity and MICROCHIP NUMBER (ISO or 15-digit microchip is required for almost all countries) is required for review before we make a health certificate appointment for your pet. Please note EU countries and UK have different rules on rabies vaccine and not all USA 3 years rabies vaccine are accepted.

  • Please note we are not responsible for contacting previous veterinarians or Petco/Vetco vaccine clinics for retrieving proper rabies certificate and that is solely pet owner’s responsibility. Not having proper vaccination certificate can and will delay your travel plans with your pet.

  • IMPORTANT:

  • Please note, due to an extremely high volume of international health certificate requests, we are operating on a first come -first served basis and may take up to 4-7 business days to get back to your email.

If your pet’s travel plans are urgent, we highly suggest seeking other accredited veterinary clinics for your pet’s health certificate if we are unable to accommodate your request.

International health certificates involve a 2-step party process; one is the accredited veterinarian and the second is the USDA office (government veterinarian). Once we submit the documents, they are submitted directly to the USDA, VEHCS web portal. Please note, the USDA office may take 3-7 business days for endorsements, and we have no control in the endorsement timeline so please plan accordingly.

    Thank you.
  • Please do not email us health certificate forms, we will fill those out at our end.
